This is the most common Thai immigration form. The TM.7 is the form you will need to complete with a photo to extend you tourist visa. The re-entry permit in Thailand is needed when you are going to leave the country and wish to keep your current visa status when you return. Also this is submitted through immigration to the Prime Minister and the Minister of the Interior. This is where you have been banned from Thailand. You will need to make your appeal on the bases of your banning order.

If your visa has been revoked you can appeal this revocation to the Immigration Commission. Its a standard application form where you will need to attach the reason why you think that the revoking of your visa was incorrect. You can change the address when you arrive in Thailand. You will need to file the TM.27 address change form. This is not a very good copy of it so it is here only for reference purposes. You would be better off getting it as an immigration office.

The TM.88 is the application for a visa on arrival. Not everyone can enter Thailand on a visa on arrival so check if you are allowed in to Thailand. They normally had this out on the airplane or buss before you enter thailand. Its blue and not the usual black and white. Best to look at it and understand it but dont use it as they might not accept a copy of the original. This is the Thai arrival card and Thai departure card. This will sit in your passport for years as you keep extending your visa without leaving the country. So it takes a lot of wear and tear.
